Requesting a Fit Note (Sick Note)
If you have been ill for more than 7 days and need a fit note (sometimes called a sick note), you can request one from our GP practice.
If you have been ill for 7 days or less, you do not need a fit note from a GP. Instead, you can complete a self-certification form for your employer. This form is available from your employer or on the NHS website.
How to Request a Fit Note
- Online Request – You can request a fit note using Klinik
- Request by Phone or In Person
- Medical Review – A GP will review your request and determine whether a fit note is appropriate. You may need to book an appointment if your condition requires further assessment
What Information is Needed when requesting online on by phone?
To process your request efficiently, please provide:
✔ Your full name and date of birth
✔ Details of your illness or condition
✔ The start date of your sickness absence
✔ Whether this is a new request or an extension of a previous fit note
Receiving Your Fit Note
If approved, we will send your fit note via text message or collect it in person, based on your preference.
If your fit note is not approved, we will inform you of the next steps.
